PLEASE GIVE THIS FORM TO YOUR HEALTHCARE PROVIDER TO SUPPLY US WITH INFORMATION CONCERNING YOUR REQUIRED TWO STEP TB TEST.

The procedure for giving the two-step TB test is as follows:

TWO-STEP TB Testing

1. Give the first test with 0.1ml of Tuberculin
2. Read the first test in 48 – 72 hours
3. If the reaction to the first test is negative , give a second test with the same dose and strength of Tuberculin a week later
4. Read the second test within 48-72 hours. If the second test is negative, no follow up is required. A yearly single step will be required.
5. If the second test is positive, as per CDC guidelines, the person is a positive reactor and will be referred to their physician for follow-up.( it is highly likely that the physician will ask for chest x-ray)
